Job Description:
- Holds a CACI (or equally recognized SA certification equivalent or higher) and assesses the need for and provides substance abuse treatment and supports for team consumers
- Provides individualized substance abuse/addictive disease treatment to an average of at least 3 individuals daily, documented with progress notes that reflect use of EBP
- Conducts weekly substance abuse groups with individuals who are dually diagnosed
- Maintains a caseload of 10-12 consumers, ensures scheduled MD appointments, and assists with transportation planning
- Follows up on missed MD appointments, with at least 80% of services delivered in the community
- Attends an average of 4 weekly treatment team meetings, participates in treatment planning
- Available for after-hours (crisis phone) coverage as assigned
- Performs related duties as assigned by the supervisor
- Maintains compliance with all company policies and procedures
Position Requirements:
- Valid Driver's License
- Experience working with individuals with Substance Abuse and/or Mental Health Diagnoses (Preferred)
- Fingerprint-Based Background Check of the Georgia and Federal Criminal History Record
- Drug Screen
- Motor Vehicle Report (MVR) for seven years
- Bachelor's Degree in a related field from an accredited college or university or certification as an addiction counselor from the Georgia Addiction Counselors Association (GACA) or equivalent
- Ability to lift and carry up to 50 lbs
- Knowledge of evidence-based practices for dually diagnosed population
- Proficiency in Motivational Interviewing
- Ability to provide integrated treatment for mental health and substance abuse
