Responsibilities
  • Conduct comprehensive assessments of clients' needs and develop individualized treatment plans in collaboration with the treatment team.
  • Provide ongoing case management services to clients, ensuring they receive timely and appropriate support throughout their residential program stay.
  • Assist clients in accessing and engaging with various services, such as medical care, counseling, vocational support, and community resources, to support their recovery journey.
  • Conduct regular progress meetings with clients to review treatment plans, identify barriers, and set achievable personal growth and recovery goals.
  • Collaborate with other professionals, including counselors, therapists, and medical staff, to ensure coordinated care for clients.
  • Maintain accurate and up-to-date client records, documenting interactions, progress notes, and treatment interventions in compliance with organizational and regulatory standards.
  • Ensure adherence to ethical guidelines, confidentiality regulations, and policies related to client care and case management.
  • Provide crisis intervention and support as needed to ensure clients' safety and well-being during challenging times.
Qualifications
  • Bachelor's degree in a social service-related field from an accredited college or university.
  • One year of experience in a social services-related field.
  • Clean driving record and the ability to pass a background check and drug test.
  • Demonstrated experience in case management, preferably in a substance abuse or mental health setting.
  • Excellent communication and interpersonal skills to effectively engage and build rapport with clients and the treatment team.
  • Strong organizational skills, with the ability to manage multiple tasks and prioritize responsibilities effectively.
