Registered Nurse (RN) Case Manager

ATC Healthcare is seeking a Registered Nurse (RN) Case Manager with a minimum of 1 year of experience for immediate hire to join our warm, safe, and friendly team.

Responsibilities
  • Assessing Patient Needs: Conduct comprehensive assessments to determine patient needs and develop individualized care plans.
  • Care Coordination: Collaborate with healthcare teams, patients, and their families to ensure timely and appropriate healthcare services.
  • Patient Advocacy: Act as a dedicated advocate for patients, ensuring they receive the best possible care and resources.
  • Monitoring Progress: Regularly evaluate patient progress and adjust care plans as necessary to achieve optimal outcomes.
  • Education and Support: Educate patients and their families about care plans, health care options, and provide emotional support.

Requirements
  • At least 1 year experience in the above field.
  • Current Registered Nurse (RN) license in the state of practice.
  • Active BLS/CPR and TB certifications.
  • Strong knowledge of anatomy, physiology, and medical terminology.
  • Experience in critical care or emergency nursing is preferred.
  • Excellent communication skills for effective interaction with patients and healthcare teams.
  • Ability to work well under pressure in fast-paced environments.
  • Experience working with children or individuals with developmental disabilities is a plus.
