Job Description:
We are seeking a compassionate Physical Therapist (PT) to join a skilled nursing and long-term care facility in Stamford, CT, on a travel contract. The ideal candidate is patient-centered, detail-oriented, and experienced in geriatric rehabilitation.
Key Responsibilities:
- Conduct thorough patient evaluations to assess mobility, function, and therapy needs.
- Develop and implement individualized treatment plans in collaboration with physicians and care teams.
- Provide therapeutic interventions including strengthening, gait training, and balance activities.
- Educate residents, families, and caregivers on therapy programs, safe mobility, and the use of assistive equipment.
- Document evaluations, progress notes, and treatment outcomes in compliance with facility and regulatory standards.
- Collaborate with interdisciplinary teams to ensure coordinated care and optimal patient outcomes.
- Promote patient independence, safety, and quality of life through evidence-based therapy.
Qualifications:
- Doctor of Physical Therapy (DPT) or Master's in Physical Therapy from an accredited program.
- Current and valid Physical Therapist license in Connecticut (or compact license).
- Minimum of 1 year of PT experience, preferably in long-term care or skilled nursing facilities.
- Current Basic Life Support (BLS) certification.
- COVID vaccine and recent TB test.
- Strong communication, assessment, and organizational skills.
- Compassionate, patient-focused approach with experience supporting geriatric populations.
