Job Description:
Saint Helens Post Acute is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) for a travel contract position. Join our team and provide essential nursing care in a professional and supportive skilled nursing environment. Duties and Responsibilities:
- Provide direct nursing care to residents under the supervision of an RN or physician
- Administer medications and treatments as prescribed and document accurately
- Monitor and report changes in resident conditions promptly
- Assist with admissions, discharges, and transfers of residents
- Collaborate with interdisciplinary team members to develop and implement individualized care plans
- Support residents with activities of daily living (ADLs) including bathing, dressing, mobility, and feeding
- Ensure compliance with infection control practices and safety protocols
- Maintain accurate medical records and documentation in accordance with facility policies and state regulations
- Communicate effectively with residents, families, and staff
- Participate in training, in-services, and staff meetings as required
Requirements:
- Active LPN license (Oregon or compact state license)
- Current BLS Certification (Basic Life Support)
- COVID-19 vaccination record
- Recent PPD test or clear chest X-ray
- Clear background check and drug screening
- Previous experience in a skilled nursing or long-term care setting preferred
