- Active Nurse Practitioner (NP) license in the state of Ohio
- Current Basic Life Support (BLS) certification from AHA or Red Cross
- Minimum of one year of recent NP experience, preferably in palliative, hospice, or home-based care
- Current Tuberculosis Screening (PPD) and physical (or ability to obtain)
