Completion Bonus: $1,895 per contract (up to $5,685 after three contracts)

Job Description

ATC Healthcare is seeking a qualified Licensed Clinical Social Worker (LCSW) or Associate-level clinician (ASW, AMFT, or MFT) for a part-time travel assignment in Sacramento, California. This role supports hospital-based behavioral health and social services for adolescent, adult, and geriatric patient populations.

The clinician will provide psychosocial assessments, care coordination, discharge planning support, and interdisciplinary collaboration to ensure safe transitions of care and high-quality patient support services in an acute care setting.

Key Responsibilities

• Provide clinical social work services in a hospital setting.
• Conduct psychosocial assessments and care planning.
• Support discharge planning and care coordination.
• Collaborate with interdisciplinary healthcare teams.
• Provide patient and family support services.
• Maintain accurate documentation in Epic.
• Participate in interdisciplinary rounds and care planning.
• Support hospital social services operations and workflows.

Qualifications

• Active California license required:
  • LCSW or
  • ASW or
  • AMFT/MFT
• Minimum 2 years of experience required.
• Epic experience preferred.
• Strong communication and interdisciplinary collaboration skills.
• Ability to work independently and as part of a care team.
