Wellness Services Direct/RN starting at 120K + Benefits

Our client is hiring a Wellness Services Director/RN for each of its two assisted living communities in the Hillsboro, OR, area. The direct hire position starts at $120K with a possible $5,000 signing bonus. Relocation is an option for the right candidate.

Shift:

This position requires working Monday through Friday 8a-5p or as needed to manage the community's health services and to otherwise be on call-willing to work shifts assigned, weekends, and holidays.

Position Summary:

The Wellness Services Director/RN will be responsible for all aspects of Health Services, ensuring that state and federal regulations are followed and resident satisfaction and care needs are met.

The Wellness Services Director/RN's responsibilities include overseeing medication administration, physician orders, incident reporting, investigations, and revisions to service plans to maintain compliance with state rules and regulations. The Wellness Services Director/RN is also responsible for nurse delegations and change of conditions.

Essential Functions:
  • Exemplifies core values in words and actions.
  • Protects resident protected health information (PHD and adheres to Community confidentiality agreement.
  • Holds team members accountable to exemplifying core values and appropriately addresses behavioral issues as needed.
  • Knowledge and understanding of state regulations and Company policies and procedures, ensuring compliance with both as it relates to responsibilities.
  • Actively and appropriately participates in state surveys and, if necessary, assists with developing and implementing the Plan of Correction.
  • Purchases needed supplies within the Health Services Budget.
  • Communicates budget status weekly at designated stand-up meetings to include expenses, staffing hours, overtime, and agency projections/use.
  • Maintains a clean and organized workspace
  • Adheres to Community dress code for Leadership Team members
  • Maintains strict confidentiality of all known sensitive information, including residents, staff, and community operations.
  • Attends and participates in the Daily Stand-up Meetings
  • Participates in manager-on-duty (MOD) rotation
  • Collaborates with other departments
  • Av ailable by phone 24/7 and makes appropriate arrangements when not available
  • Purchasing within budget utilizing the expense management tools
  • Leads and/or participates in investigations and documentation as needed.
  • Participates in selecting and recognizing the Employee of the Month and other efforts to enhance employee engagement.
  • Help promote the Community through positive relationships with outside agencies and local organizations.
  • Responds to, leads when necessary, and carries out responsibilities during emergencies/disasters per the Community's Emergency/Disaster Plan.
  • Participates in monthly all staff meetings and conducts in-services.

Leadership
  • Recruits, hires, orients, trains, supervises, and manages the health services staff to include the LPN, Compliance Specialist, Resident Care Coordinator, Health Services Assistant, Caregivers, Lead Medication Assistants and Medication Assistants.
  • Ensure completion of orientation and skills checklist.
  • Responsible for the performance of Health Services Staff - providing ongoing feedback and coaching/counseling when appropriate to include leading or participating in employee terminations as necessary.
  • Schedules & ensures sufficient staffing of Care Partners & Medication Assistants, according to/in compliance with the staffing budget and the needs of the residents
  • Notifies the Administrator when staffing budget is not sufficient to meet residents needs to evaluate need for increase.
  • Assists on the floor as needed.
  • Regularly visits the Community during all shifts to build relationships and encourage employee engagement, supervision, and address performance concerns.
  • Uses staffing agencies only with approval of Administrator on a limited time basis.
  • Investigate incidents and issues that come up involving employees and documents per policies and procedures

Residents

Manages move-in process from time of reservation to move-in to include the following tasks:
  • Obtains completed Physician paperwork. Assess the prospect in a timely manner at the prospect's residence, including reviewing the physician's paperwork, interviewing those involved in the resident's care, and making observations.
  • Complete the move-in consultation form and notify the administrator when a prospect qualifies for such a position.
  • Communicates service level costs or denial to Prospect/Representatives.
  • Develops a Care Plan and prepares Care Partners and Medication Assistants for resident's arrival - helping to ensure a positive experience on the first day of residency.
  • Participates in orienting residents and their families to help ensure expectations can be met and resident/families know who to contact for what is known, and to help them understand Community protocols.
  • Develops thorough Care Plans with the assistance of residents and those involved in their care. Re-assesses and meets with residents and those involved in their care every 6 months.
  • Ensures care plans/service schedules reflect the actual care/services being provided to the resident and that all care/services provided are incorporated into reassessment to ensure the resident is charged at the correct care level.
  • Ensures all Care Plans and Service Level Agreements are signed by resident and/or legally authorized representatives.
  • Develops & implements interim care plans for acute situations that do not qualify as a significant change of condition. Ensures resident services are provided as identified in the Care Plan.
  • Coordinates care when the Community cannot meet the needs/expectations of the residents or the resident would benefit from outside agencies such as Home Health Agencies, Hospice Agencies, Companion Care Agencies, Transportation Services etc.
  • Ensuring residents, resident's primary contact and Physician is aware of resident's health/wellness, incidents, significant changes etc.
  • Ensure completion of Incident Reports per policies and procedures. Investigate resident incidents, reports of missing items, suspected resident abuse, or other resident related issues. Communicates results of investigation to Administrator, then to resident/resident's representative. Documents investigations and results of investigation per policies and procedures.
  • Works with First Responder and Administrator to report reportable incidents per state regulations.
  • Maintains positive relationships with families and physicians to ensure the best possible care for the residents and continues to show goodwill toward the Community.
  • Develops Negotiated Risk Agreements, obtains approval from the Administrator, and presents to residents/families when the resident's choices warrant such an agreement.
  • Alerts Administrator of residents whose needs are believed to exceed the scope of our license and/or ability to meet the resident's needs and assists with processes for transfers to a more appropriate setting.
  • Maintains complete, accurate, and thorough resident records per policies and procedures.
  • Ensures Physician Order is present for all medications/treatments dispensed/assisted with, including over-the-counter medications, medicated barrier creams, herbal supplements, vitamins, and other supplements.Ensures proper storage and controls for narcotic medications to include shift-to-shift narcotic count. Responds to and investigate missing narcotics at the time of discovery.
  • Ensures discontinued and expired medications/treatments are secured and stored in designated areas and then destroyed per policy.
  • Coordinates with pharmacies to provide flu & pneumonia vaccination clinics annually.
  • Coordinates with the Dining Services Director to provide special diets for residents according to Care Plan and Physician Orders, as allowed by policies and procedures.
  • Coordinate with the Life Enrichment Coordinator to incorporate the resident's Individual Engagement Plan into the Care Plan.

Compliance & Quality Assurance
  • Performs quality assurance audits as needed and per schedule in policies and procedures

  1. Works with health services staff to correct issues and ensure compliance/accuracy. Documentation as identified in an audit.
  2. Regularly observing Medication Assistants and Care Partners to ensure compliance, positive work habits, and that responsibilities are carried out accurately.
  3. Maintains positive working relationship with the Community's preferred pharmacy, documenting & reporting issues/errors and resolutions timely.
  4. Leads Quality Assurance and Performance Improvement (QAPI) Committee.

To perform other duties that may be set forth by Community Administrator, completing all assigned duties which may change from time to time according to resident's needs, staffing levels and working circumstances.

Physical Requirements:

This is a physically active position. Ability to walk throughout the community continually throughout the day. Push and pull wheelchairs and other equipment up to 300 pounds. Physical ability to assist residents weighing up to 300 pounds. Visual and auditory acuity within normal limits. Bending, kneeling, and occasionally reaching above the shoulder level and carrying up to 50 pounds. Ability to sit for short periods. Ability to use handheld devices and computers/software, including Yardi.

Other Requirements:

Exceptional verbal and written communication skills. English skills adequate to allow communication with residents and staff and to understand written and verbal instructions. Attention to detail and problem-solving skills. Must have compassion for the elderly. Must be comfortable with technology and the use of software programs. Familiar with the Health Insurance Portability and Accountability Act (HIPAA). Ability to multi-task and prioritize, as well as excellent time management skills. Ability to be a contributing member of a team. Demonstrate excellent customer service skills. Must have patience, tact, cheerful disposition, and enthusiasm, as well as be willing to handle resident complaints and grievances.

Travel Requirements: Regular and frequent local travel is necessary. Mileage is reimbursed at the current IRS rate.
