Summary
We are seeking a dedicated Registered Nurse-Case Manager to join our team at ATC Healthcare. As an RN Case Manager, you will play a pivotal role in managing and coordinating the care of patients with complex or chronic medical conditions.
Responsibilities
- Assessing Patient Needs: Conduct comprehensive assessments to determine patient needs and develop individualized care plans.
- Care Coordination: Collaborate with healthcare teams, patients, and their families to ensure timely and appropriate healthcare services.
- Patient Advocacy: Act as a dedicated advocate for patients, ensuring they receive the best possible care and resources.
- Monitoring Progress: Regularly evaluate patient progress and adjust care plans as necessary to achieve optimal outcomes.
- Education and Support: Educate patients and their families about care plans, health care options, and provide emotional support.
Requirements
- At least 1 year experience in the above field.
- Current Registered Nurse (RN) license in the state of practice.
- Active BLS/CPR and TB certifications.
- Strong knowledge of anatomy, physiology, and medical terminology.
- Experience in critical care or emergency nursing is preferred.
- Excellent communication skills for effective interaction with patients and healthcare teams.
- Ability to work well under pressure in fast-paced environments.
- Experience working with children or individuals with developmental disabilities is a plus.
