ATC Healthcare Services, LLC of Nassau County is seeking a Case Manager for the staffing needs of our client. Our client has an IMMEDIATE NEED for a Full-time Case Manager.
The Case Manager will ensure the physical and mental wellbeing of clients. They will be responsible for overseeing client's cases from intake to move out, providing the support necessary to achieve the best outcomes. The Case Manager will report to Case Manager Supervisor/DSS.

LOCATION: Queens, NY
Case Manager will be responsible for:
- Carrying out case management activities as per the Part 900 Regulations and the Department of Homeless Services (DHS) Case Management Guidelines for family shelters.
- Completing an assessment and fathers required documents (i.e.: birth certificates, social security cards, picture IDs, etc.)
- Showing the Safe Sleep Video and carrying out the appropriate documentation process.
- Making sure that clients sign the Code of Conduct and the facilities rules and regulations.
- Obtaining any possible proof of employment and assisting with obtaining entitlements.
- Maintains clients' records by reviewing case notes, logging events and progress.
- Communicating clients' progress by conducting weekly interdisciplinary meetings and evaluations.
- Handling intakes of adults and ensures that all documentation is accurate and properly filed.
- Recommending action plan and developing an Independent Living Programs (ILP) for clients that is reviewed and updated on a weekly basis.
- Collaborating with the Housing Specialist to create an exit strategy and target exit date.
- Tasks family with seeking and accepting housing.
- Responsible for meeting target goals set for each client by the Housing Specialist
- Conducting weekly unit inspections.
- Maintaining detailed records of each client in the form of progress notes through CARES.
- Providing Director of Social Services (DSS)/ Case Manager Supervisor with reports and summaries of caseload for management review.
- Liaison for management, communicating clients' progress and any case concerns.
- In the event of an emergency, Essential Employees are required to report to work as scheduled.
- Duties and responsibilities are subject to change based upon assessment of company needs.
Case Manager Qualifications:
- Bachelor's degree in psychology, Social Work, Sociology, Mental Health, Nursing, Human Services, or Education Services required.
- Minimum of 2 years' experience working within the Social Services Field
- Must have strong interpersonal skills and be able to communicate effectively (verbally and in writing)
- Must be organized.
- Experience with DHS Cares / Homeless population is a plus.
- Bilingual English/Spanish is a plus.
