Wednesday: 8:45-4:45 pm
Thursday: 8-5 pm
Friday: 11-5pm
PAY:
$ 51.00 per hour

GENERAL SUMMARY
The Social Worker, as a member of an interdisciplinary care team, addresses the psychosocial needs of clients and their families, assists clients and families in adjusting to the rehabilitation process and extended care living, assists clients in maintaining their rights, and works in conjunction with the care team, family, and community in providing opportunities for restorative or adaptive rehabilitation. Active participation is required in facility in-services and educational programs. The Social Worker provides quality client care considering age-specific, developmental, and cultural needs through competent practice. Demonstration of designated competencies is required.

- Master's degree in Social Work from an accredited college or university.
- Professional license as required by the State of Michigan.
- CPR certified.
- Valid driver's license.

ESSENTIAL DUTIES AND RESPONSIBILITIES

- Participates as a member of the interdisciplinary team to develop interdisciplinary care plans for clients served by that team.
- Identifies medically-related social and emotional needs of clients and families, and develops and implements programs/ services to meet those needs.
- Assists with admission and discharge procedures.
- Documents Social Work activity as required by local, state, and federal rules and regulations and as needed to meet the needs of the interdisciplinary team, the client, and the family. The Social Worker also maintains confidential records for sensitive material not appropriate for the client's chart.
- Manages crisis intervention with clients and families as appropriate.
- Provides information to clients and families regarding Advance Directives, Guidelines for Life Sustaining measures and Client Rights.
- Provides client, family, and staff education and training as needed and documents appropriately.
- Timely documentation of client assessments, progress notes, care plans, discharge summaries, and outcomes data in accordance with existing criteria.
- Participates in Care Conferences and Team Meetings and communicates with members of the team concerning client treatment as necessary.
- Maintains good rapport and cooperative relationships with team and approaches conflict in a constructive manner. Helps to identify problems, offer solutions, and participates in their resolution.
- Maintains confidentiality of information pertaining to clients, staff, and business operations, and adheres to all HIPAA standards.
- Behaves in accordance with the Mission and Values of Resilire Neuro.
- Assumes responsibility for performance of job duties in the safest possible manner, to assure personal safety as well as that of clients and co-workers; including safe transportation of clients in the community.
- Completes required education & training learning hours annually and per licensing body if applicable.
- Respects and appropriately considers age, gender, cultural background, and related factors when providing services to all internal and external customers.
- Demonstrates ability to work appropriately with clients who may exhibit aggressive or difficult behaviors following behavioral training.
SUPPLEMENTAL DUTIES AND RESPONSIBILITIES
- Maintains knowledge of current trends and developments in the field by reading the appropriate literature and attending professional conferences.
- Participates in facility committees/program planning as time, interest and necessity dictate.
- Participates in Quality Improvement/Quality Assurance programs as applicable.
- Organizes and participates in educational activities for other health professionals, clients, and their families.
- Maintains contact with community agencies, Institutions and organizations, and keeps a file on community resources available to aid clients and families.
REQUIRED SKILLS AND ABILITIES
- Strong written, verbal, and interpersonal skills to communicate effectively with clients, families, physicians, and other interdisciplinary team members.
- Strong interest and ability in updating skills and knowledge of advancements in the field of Social Work.
- Strong interest in working closely with clients and their families.
- Analytical skills necessary to assess clients, develop recommendations, and provide treatment as the client progresses in the rehabilitation program.
- Flexibility and adaptability; must be able to assume the role of therapist and educator.
- Good judgment and problem-solving.
- Basic computer skills for documentation and data entry.
WORKING CONDITIONS
- Work is a combination of sedentary and physical activities completed in a normal office and client care environment.
- Travel off site and transport of clients with company vehicles may be required.
- The Social Worker is frequently exposed to verbally and physically aggressive clients.
- Exposure to Bloodborne Pathogens, infectious material and chemicals is possible.
