Physical Therapist (PT) – Post-Acute Care - Travel Position (with $7,500 Completion Bonus)

Pay: $2,635.20/week (plus $2,500 completion bonus per contract, up to $7,500 after three contracts

About the Role

We are seeking a dedicated Physical Therapist to join our post-acute care team. The PT will play a vital role in helping patients recover from illness, injury, or surgery by restoring mobility, strength, and independence. This position involves working closely with an interdisciplinary team to deliver high-quality rehabilitation services.

Key Responsibilities

• Perform comprehensive physical therapy evaluations for patients in a post-acute setting.
• Develop and implement individualized treatment plans based on clinical assessments and patient goals.
• Provide therapeutic interventions including exercises, manual therapy, and use of modalities.
• Collaborate with nurses, occupational therapists, speech therapists, and physicians to ensure coordinated care.
• Educate patients and families on therapy goals, techniques, and home exercise programs.
• Maintain accurate documentation of treatments and progress in compliance with regulatory standards.
• Adhere to all facility policies, state, and federal regulations.

Qualifications

• Education: Doctorate or Master’s degree in Physical Therapy from an accredited program.
• Licensure: Current Physical Therapist license in the state of practice.
• Experience: Prior experience in post-acute or skilled nursing facility preferred.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively in a team environment.
