Position Summary:

 

We are seeking a compassionate and dedicated Licensed Social Worker (LSW) to provide psychosocial support, counseling, and care coordination services to patients and their families. The ideal candidate will assess patients' social, emotional, and environmental needs and collaborate with interdisciplinary healthcare teams to develop and implement effective care plans.

Key Responsibilities:

 

  • Conduct comprehensive psychosocial assessments of patients and families.
  • Develop, implement, and monitor individualized care and discharge plans.
  • Provide counseling, crisis intervention, and emotional support services.
  • Coordinate community resources and referrals to support patient needs.
  • Advocate for patients and families to ensure access to appropriate services.
  • Collaborate with physicians, nurses, case managers, and other healthcare professionals.
  • Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements.
  • Assist patients with transition planning, including discharge and post-discharge support.
  • Educate patients and families regarding available resources, benefits, and support programs.
  • Participate in interdisciplinary team meetings and case reviews.

Qualifications:

 

  • Bachelor's or Master's Degree in Social Work (BSW or MSW) from an accredited institution.
  • Current and active Licensed Social Worker (LSW) license in the state of practice.
  • Minimum of [1-2+] years of social work experience preferred.
  • Experience in healthcare, hospital, long-term care, rehabilitation, behavioral health, or community settings preferred.
  • Strong assessment, counseling, and case management skills.
  • Excellent communication, interpersonal, and organizational abilities.
  • Knowledge of healthcare regulations, community resources, and discharge planning processes.
