Contract Type: Travel
Duration: 13 weeks
Shift: 4x10 Days
Pay: $2,600/week
Completion Bonus: $2,500 per contract (up to $7,500 after three contracts)

Job Description

Travel Physical Therapist needed for a home health setting providing therapy services to patients in their homes. Schedule is Monday through Friday with approximately 40 hours per week and an average of 5 patient visits per day. Therapist will travel between patient homes and receive mileage reimbursement. Charting systems used include Netsmart and Epic. Candidates must be quick learners, self-starters, and able to ramp up quickly. Housing in the area can be challenging, especially for travelers with pets.

Key Responsibilities

• Provide physical therapy evaluations and treatments in patient homes
• Manage an average caseload of about 5 visits per day
• Develop and implement individualized care plans
• Document visits and care plans in Netsmart and/or Epic
• Coordinate with interdisciplinary care teams
• Travel between patient locations and track mileage
• Maintain compliance with home health and safety standards

Qualifications

• Active Oregon PT license required at time of submission
• BLS certification required
• Home health experience preferred
• Ability to work independently and learn systems quickly
• Strong time management and patient care skills
