Social Worker – Hospital Case Management

Shift: 5×8 Days (Flexible start between 8:00–9:00 AM, Monday–Friday)
Pay: $2,384/week
Completion Bonus: $2,500 per contract (up to $7,500 after three contracts)

Job Description

ATC Healthcare is seeking an experienced and highly skilled Social Worker to support care transitions in an acute care hospital setting. This role collaborates closely with the Interdisciplinary Care Transitions (ICT) team to assess psychosocial needs, coordinate discharge planning, and advocate for patients throughout the continuum of care.

The Social Worker will provide ongoing support through comprehensive assessments, development of individualized treatment plans, case management, mediation, and resource coordination. This position plays a vital part in ensuring safe transitions, optimizing patient satisfaction, and addressing complex psychosocial, emotional, and environmental factors impacting patient health.

Key Responsibilities

• Conduct psychosocial assessments and develop individualized treatment and transition plans.
• Collaborate with the Interdisciplinary Care Transitions (ICT) team to coordinate and support patient care needs.
• Facilitate discharge planning, including hospice, palliative care, IHSS, FMLA/disability documentation, and advance directives.
• Provide ongoing consultation, mediation, patient advocacy, and resource coordination.
• Support patients and families in navigating emotional, psychological, and environmental challenges impacting care.
• Educate staff on psychosocial considerations, discharge planning, and care coordination practices.
• Monitor patient progress and continually evaluate and update treatment plans.
• Document interventions and maintain accurate patient records in accordance with facility policy.
• Participate in reviewing and investigating adverse occurrences as needed.

Qualifications

• Master’s degree in Social Work, Marriage and Family Therapy, or Professional Counseling (required).
• Active California license in Social Work or related field (required).
• Minimum 3 years of recent hospital-based social work experience (required).
• Experience with hospice, palliative care, code status discussions, advance directives, IHSS, emergency visas, and FMLA/disability paperwork (required).
• Current BLS (AHA only).
• Strong clinical assessment, crisis intervention, and discharge planning skills.
• Ability to collaborate with multidisciplinary teams and manage complex patient cases.
• Previous travel experience preferred but not required.
• Proof of COVID-19 vaccination and recent tuberculosis test required.
